Professional and self-referrals are both welcome. Complete the online form and our team will be in touch to discuss next steps.
Before you submit
Having the following information ready will help us process your referral as quickly as possible.
- NDIS plan details: plan number, plan dates, and funding category (if applicable)
- Medicare details: GP Management Plan or Mental Health Care Plan (if applicable)
- TAC claim number and contact details (if applicable)
- Any existing reports: paediatric, psychological, speech pathology, or school reports
- Child's date of birth and Medicare/healthcare card number
- Parent or carer contact details and preferred contact times

Whether you're a GP, paediatrician, allied health professional, teacher, or a parent/carer referring directly, use the same secure online form. All referral types are welcome.
